Please check the box below agreeing to the Asian International Film Festival Arizona's Release and Waiver of Liability: I agree to complete the volunteer shift(s) at the Asian International Film Festival Arizona (hereinafter “AIFFA”) that I have been assigned and will attend orientation and complete all training as required without compensation or promise of future employment. I will support a professional attitude, appearance, and will maintain a high work standard in my interactions with patrons, staff and volunteers. I understand that the activities assigned may involve using personally owned transportation (my own vehicle), standing for extended periods of time, lifting materials ranging from light to heavy, walking in darkened rooms such as a theater, and interacting with members of the public. I hereby waive, release and forever discharge (on my own behalf and on behalf of my heirs and personal representatives) any and all rights of claims I may now or hereafter have against AIFFA, and the City of Tempe including but not limited to, any claims of bodily injury, personal injury, illness, death or property damage that may result from my activities with AIFFA, whether caused in whole or in part by the negligence oF AIFFA or its officers, directors, employees or otherwise. I consent to all emergency medical treatment as may be deemed necessary and/or appropriate under existing circumstances by medical personnel as needed and understand that AIFFA does not provide or carry, and does not have any responsibility to provide or carry, liability, medical, health or disability insurance in the event of an injury. AIFFA does maintain public liability insurance for all events at all times. Photography: I hereby grant and convey unto AIFFA all rights, title and interest in any and all photographic images and video or audio recordings and digital imaging via any and all media now known or unknown as of the date of this release made by AIFFA or at AIFFA'S direction of me during my activities with AIFFA, including, but not limited to, any royalties, proceeds or other benefits derived from such photographs or recordings, in perpetuity. I authorize the references I have provided to give the Festival any and all information concerning my previous employment regarding any pertinent information they may have, personal or otherwise, and release all parties from any liability for damages that may result from furnishing the same to the Festival. I expressly agree that this release is intended to be as broad and inclusive as permitted by the laws of the State of Arizona and that this release shall be governed by and interpreted in accordance with the laws of the State of Arizona. I agree that in the event that any clause or provision of this release is held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining portions of this release which shall continue to be enforceable. I understand that if I am under 18 years old, I must have a consent form signed by a parent/guardian to participate.
